By filling in the form below, you can easily and anonymously report violations, suspected illegal actions, acts of corruption executed by the staff of the Siauliai City Municipality etc. The information provided by you will be registered and examined if it is within the competence of the Municipality. The form below must be submitted in Lithuanian. Provision of information to the institution is free of charge. The person providing the information may or may not provide his or her name and/or contact details. If you provide your contact details (phone number and/or email address), you will be directly informed of the progress of your inquiry and the resulting decision. It is important to note that the Director of the Siauliai City Municipality Administration, a person authorized by the Director, or another entity established by legal acts has the right to make a decision not to examine the information submitted to the institution anonymously. At the request of the applicant, the confidentiality of the person and of the information is ensured.
